File Report Request Order (draft order)

This Quick Reference Guide shows how to file a Report Request Order (draft order).
File Clinic Report Request Order (draft order)
1. From CMS Portal Home page, click: File on One of My Cases. List of cases display.
Note: Only cases user previously filed on or requested access to, display.

2. For relevant case, click case number link. Filings tab displays.
3. Click Hearings tab. Hearings tab displays.
4. For relevant hearing, click: Add Draft Order. Add Draft Order screen displays.
5. To identify filing, type reference in My Reference field.
6. Complete required panels and fields.
Order Panels
- Consent Type: Select relevant option.
- Hearing Notations: Type if required.
- Sub-cases: Select relevant subcase.
- Add Order(s): Select: Report Request.
7. Click plus icon. Report Request Order panel displays.
8. In Report field, select: Clinic. Further fields display.
9. Complete required panels and fields.
Panel: Report Request Order
- Request to: -
- Name & Relationship to child: Type relevant name(s).
- Unless the Clinician determines otherwise, the report to contain: Deselect if not required.
- The Court requests the Clinician to include in the report an opinion/ recommendation as to:
- Select relevant option(s).
- Note: If the following additional specific matters is selected, free text Details field displays.
- The Court requests the Clinic to conduct a neuropsychological assessment of:
- Select if required.
- Note: If selected, type name of relevant party(s).
- Reason: Type reasons.
- Reference material:
- Select relevant option(s).
- Note: Additional fields display when some options selected.
- Due Date: Type date or click Calendar icon to select date.
10. Click: Add Order. Order Summary displays:
Note: Once all orders are added, click Preview button to review order in more detail.
11. Is another order required?
- Yes:
- Add order.

- No: Go to next step
12. Is Order Summary correct?
- Yes: Go to next step
- No:
- Amend order

13. In Share with parties or submit to court? field, select relevant option.
Note:
- Share with Parties allows other parties to view and update Minutes of Proposed Orders
- Minutes of Proposed Orders cannot be updated once Submit to Court selected
14. Click: Proceed. Confirmation of Filing screen displays.
Note:
- Minutes of Proposed Orders can be updated in Orders tab any time before submission to Court
- Once submitted, Minutes of proposed orders document generates and can be accessed through My Account screen

File DFFH Report Request Order (draft order)

3. Click Hearings tab. Hearings tab displays.

5. To identify filing, type reference in My Reference field.

Order Panels

Panel: Report Request Order
- Report:
- Select either:
- Conciliation Conference Addendum
- Disposition
- Existing disposition reports to stand under s557(1)(d)
- Further Disposition
- Protection
- Readiness Hearing
- Temporary Assessment Order
- Therapeutic Treatment Order progress
- Note: Selection may alter which fields display.
- Select either:
- Further disposition report in short form?
- Select Yes or No.
- Note: Field displays when Further Disposition report requested.
- Request details: Type details.
- Due Date: Type date or click Calendar icon to select date.
9. Click: Add Order. Order Summary displays:
Note: Once all orders are added, click Preview button to review order in more detail.
10. Is another order required?
- Yes:
- Add order.

- No: Go to next step
11. Is Order Summary correct?
- Yes: Go to next step
- No:
- Amend order

10. Click: Add Order. Order Summary displays:
Note: Once all orders are added, click Preview button to review order in more detail.
11. Is another order required?
- Yes:
- Add order.
