Forms
3 types of forms can be filed in Portal:
- Case Initiation
- Subcase (Application)
- Document
SMART FORMS
The following CMS-generated forms or 'smart' forms can be filed in Portal without pre-preparing a document:

| AAVLC | Notice of application under section 42M, 42N or 42P of the Evidence (Miscellaneous Provisions) Act 1958 |
| ABAI | Application for bail |
| ADNPP | Application for declaration to be deemed not to be a prohibited person |
| APHYC | Notice of application under section 42L of the Evidence (Miscellaneous Provisions) Act 1958 |
| APPFCV | Application for an unopposed bail variation on the papers |
| AVRNCS | Application to vary or revoke a non-custodial supervision order |
| AVRS | Application to vary or revoke a sentence |
| COMEX | Application for compulsory examination |
| CREH | Application for a rehearing |
| ERFC | Endorsement regarding order for fingerprinting a child |
| IPR | Infringement penalty for registration |
| NTACC | Notice to appear before the court |
| RROCAY | Application for review of a registrar's order |
| VAEO | Application to vary alcohol exclusion order |
| WITSUM | Witness summons |
Once filed, Portal generates the form/document as a PDF, which is either
- automatically issued, or
- sent to a Registrar to review and issue.
Depending on the process; you may be required to file additional documents.

OTHER FORMS
If you cannot locate the form name during your filing process, the form must be filed directly with the appropriate venue.
