File Temporary Assessment Order application

This Quick Reference Guide shows how to file a Temporary Assessment Order application.
1. From CMS Portal Home page, click: Children's Court of Victoria. Select a Case Type screen displays.
2. Click: Child Protection
3. Click: File Other Application. Child Protection - Other Applications screen displays.
4. To identify filing, type reference in My Reference field.
5. Complete required panels and fields.
Panel: Filing Details
- Court Location: Select court location
- Application Type: Select: Temporary Assessment Order
- Without Notice: Select Yes or No
- Link to Existing Sibling Case(s): Change if required. Note: If Yes, complete additional fields.
6. Click Next Step or alternatively click the green buttons to navigate between steps at any time.
7. Complete required panels and fields.
Panel: Applicant Details
- Surname/ Single Name: Type surname. Select correct option from results.
8. Click: Next Step
9. Complete required panels and fields.
Panel: Child Details
- Party Designation Type: Do not change
- DFFH ID: Type DFFH Client ID or person identifier.
- Surname / Single Name: Type surname or single name.
- First Name: Type first name
- Middle Name (s): Type middle name(s) if required.
- Date Of Birth: Type date or click calendar icon to select date.
- Sex/Gender: Select relevant option
- Identified term and/or pronouns: Type additional details if required.
- Indigenous Status: Select Indigenous status.
- Is an interpreter required? Select Yes or No. Note: If Yes, complete additional fields.
Panel: Child's Home Address
- Address Type: Do not change.
- Address: Type address. Select correct option from results.
- Are these address / contact details confidential? Change if required.
10. Click: Next Step
11. Complete required panels and fields.
Panel: Details of Application
- Secretary applies for: Select relevant option(s). Note: If Other directions or conditions selected, type details.
12. Click: Next Step
13. Complete required panels and fields.
Panel: Party Details (1)
- Party Role in Application: Change if required
- Relationship to Child: Select relevant option.
- Is the Party Known?: Change if required. Note:
- If party is Unknown, subsequent Party Details panels do not display.
- If a party is Deceased:
- Date of Death field displays
- Contact Details, Address and Custody panels do not display
- DFFH ID: Type DFFH Client ID or person identifier.
- Party Designation Type: Change if required. Note: If Person -Single name selected, First Name and Middle Name (s) fields do not display.
- Surname / Single Name: Type surname or single name.
- First Name: Type first name
- Middle Name (s): Type middle name(s) if required.
- Date Of Birth: Type date or click calendar icon to select date.
- Date Of Death: Type date or click calendar icon to select date. Note: If Deceased s
- Sex/Gender: Select relevant option
- Identified term and/or pronouns: Type additional details if required.
- Indigenous Status: Select Indigenous status.
- Is and interpreter required? Select Yes or No. Note: If Yes, complete additional fields.
Panel: Home Address
- Address Type: Do not change.
- Address: Type address. Select correct option from results.
- Are these address / contact details confidential? Change if required.
Panel: Contact Details
- Mobile Number: Type mobile number if provided.
- Home Number: Type home number if provided.
- Work Number: Type work number if provided.
- Email Address: Type email address if provided.
Note: Mobile Number and Email Address allow electronic notifications from CMS.
14. Complete Parent 2 Details panel.
15. Click: Next Step
16. Complete required panels and fields.
Panel: Hearing Details
- Add New Hearing:
- Select: Add New Hearing.
- Select relevant option from Listing Widget
- Court Location: -
- Hearing Type: -
- Date and Time: Do not change.
- Duration: Do not change.
- Resources: Select if required
- Hearing Notes: Type if required
17. Click: Next Step
18. Complete required panels and fields.
Panel: Additional Documents
- Code / Name:
- Type name
- Select additional document(s) if required.
- Upload Document: Click: Chose File and attach PDF document.
19. Select Electronic Signature Consent to consent to affixing signature to filing.
20. Select: Proceed. The Confirmation of Filing screen displays.
Note: Application for Temporary Assessment Order document is generated and accessed through My Account screen.
